To get the EIN (IRS)

The IRS requires the owner's SSN or ITIN (the company's responsible party). It's stored encrypted, only our office sees it and it never goes in emails. It can be deleted once the EIN is issued.

Vehicles for the USDOT

Same as Motus: how many of each type and whether they're owned or leased. Enter 0 if none.
Owned Term leased Trip leased
Straight trucks
Truck tractors
Trailers
Trip leased = for a single trip; term leased = lease of 30 days or more.

Relationship with other transportation companies

Do you or your company have, or have had in the last 3 years, a relationship with another FMCSA-registered transportation company? For example: the same owners or partners, the same management, or a relative with a trucking company. Answering yes doesn't stop you from getting your USDOT.

UCR

If you'll operate in 2027, register 2027. If you're operating now and didn't pay the current year, register the current year.

Drug & Alcohol Clearinghouse

The account uses Login.gov: it must be an email the owner checks.
Does the owner have a CDL and drive?
Drivers with a CDL (if any)
Testing consortium (C/TPA)
If the owner drives their own truck (owner-operator), FMCSA requires being in a random testing consortium.
Do you already have a clinic for DOT drug and alcohol tests?
Do you already have a Clearinghouse account?

3. Trucks

The VIN is on the title or the truck door. The weight is the maximum gross weight (truck + trailer + load), e.g. 80,000 lbs. The package includes IRP and 2290 for 1 truck; each extra truck is added to the total.

Truck 1

4. Anything else (optional)

We ask for documents (license, truck title, insurance) after payment; you can upload them in "Track my order".
